Congratulations, Dr. Aaron Jacobs!

UNM Medical Group, Inc., Names New Associate Medical Director For Population Health Management

Aaron Jacobs, MD, has been appointed as the associate medical director for Population Health Management at UNM Medical Group, Inc., effective June 1, 2019.

Jacobs is a highly respected health system physician leader, educator and clinician, who has been with UNM since starting his pediatric residency in 2000. He is board certified in pediatrics and in preventive medicine (clinical informatics). He has served in leadership positions for the past 13 years, within the Departmenbt of Pediatrics and at the UNM Children's Hospital, as well as in his most recent role as chief medical informatics officer for UNM Hospital.

"I truly appreciate the experience and the relationships that I have developed in working with the Clinical Informatics teams over the past several years," Jacob said. "As health care evolves, maintaining and improving the health of our patients and communities will be a challenge we can meet through teamwork, creativity, vision and data-driven decision making. I am eager to contribute to the Population Health team that is helping to forge this path."

In his new role, Jacobs will work under the supervision of Jami Frost, MD, the executive medical director of Population Health Management for the UNM Health System. He will focus on operationalizing value-based contracts; provider and facility performance metrics related to value-based care; design and implementation of initiatives in promoting interoperability and federal quality programs, including MIPS and MACRA; and development of UNM Team Health care coordination and chronic care management programs and population health initiatives.

Frost and Jacobs will work in close collaboration with Robb McLean, MD, the medical group's chief medical officer.
